Premises licence and Summary
The premises licence holder must ensure the premises licence (or a certified copy) is kept at the premises and that the summary of the licence (or a certified copy of that summary) is prominently displayed on the premises. A premises licence providefor the licence to:
- specify the name and address of the holder;
- include a plan of the premises;
- if the licence has effect for a limited period, specify that period;
- specify the licensable activities for which the premises may be used;
- if the licensable activities include the supply of alcohol, specify the name and address of the individual who is the premises supervisor in respect of the licence;
- specify the conditions.
Where a premises licence or summary is lost, stolen, damaged or destroyed, the holder of the licence may apply to the relevant licensing authority for a copy of the licence or summary.
Notification of change of name or address: The holder of a premises licence must, as soon as is reasonably practicable, notify the relevant licensing authority of any change in name or address, both for himself or DPS. Not complying is an offence liable to Level 2: £500
